Our office is conveniently located on Scottsdale road,
just across the street from Terravita in North Scottsdale. There is ample free
parking and we are wheelchair accessible.
Our office hours are 7:30 AM to 3 PM Monday through Friday.
There is a provider on-call after hours for emergencies. Complete and bring

We accept most insurance plans including Blue Cross Blue Shield, United Health
Care, PHCS, CCN, Mayo, CIGNA (HMO and PPO), Humana PPO, Arizona Foundation, Great West, Lifewise,
Aetna PPO,
Pacificare PPO, Worker's Comp and Medicare.
